Service closures could prompt rise in out-of-hospital births, putting pressure on unprepared volunteer ambos

Paramedics are attending births and gynaecology-related emergencies more than once a day in country WA, sparking concerns volunteer ambulance officers are not well enough trained to respond to them. A recent Edith Cowan University study revealed volunteers across Australia were not properly equipped to deal with emergency births. ECU research associate Michella Hill spoke to 28 volunteers — 18 in WA — and concluded a lack of training and equipment and logistical challenges were the main issues in delivering obstetrician services. Distance from a medical centre able to take on a patient in labour has also compounded ambulance officers’ anxieties. This is of particular concern in WA, where several maternity services have been closed, including the South West’s last remaining private maternity service at St John of God Bunbury hospital, which shut in late June. More than 200 mothers have been forced to travel to Geraldton or Perth to give birth since Carnarvon’s maternity services were closed in 2022. But the issues are growing in the city too — SJOG Subiaco in August curtailed its maternity services and was not accepting new bookings for deliveries in September, October, and November. The three-month freeze on new patients at Subiaco followed SJOG Health Care transferring its Mt Lawley obstetric services to the hospital from December 20, 2023. Staff shortages were blamed for the closure of maternity services at Ramsay Health’s Glengarry Private Hospital in Duncraig. With all of those closures across the State Australian College of Midwives president, Dr Zoe Bradfield, warns an increase in births out-of-hospital could add even more pressure to ambulance officers. Dr Bradfield said the closures would prompt an uptake of at-home and roadside births, putting further pressure on volunteer ambulance officers. “Unintended births at home and roadside births do increase when local birthing services are unavailable,” she said. “In addition, in the absence of preferred model of care, some women choose to freebirth without a health professional in attendance, which increases risks for mothers and babies. “Critical for rural areas, it’s essential to ensure that volunteer ambulance officers have access to training and upskilling.” She said an additional risk was women presenting to hospitals expecting maternity care but being met with nurses and doctors not skilled in delivering babies. Dr Bradfield said there had been reported cases of women presenting for birthing care in rural areas like Katanning, where there is a hospital but no longer a birthing service. “Katanning has a thriving multicultural community; women who have relocated from their country of origin cannot understand how we can have a functioning and capable hospital but not give birth there,” she said. “When towns lose their midwives, they don’t just lose a health professional, it has tangible intergenerational impacts.” WA midwife and paramedic Melissa Gardiner, who created a helpline to support St John WA employees through births and other obstetric emergencies, said attending births was a concern for not only regional and remote WA volunteers but also paramedics based in Perth. “I’m a paramedic of 22 years and a midwife of 13 so I’ve known for a long time that there’s a real lack of experience, education and skill and knowledge when it comes to attending obstetric patients in the pre-hospital environment,” she said. Ms Gardiner said a reason volunteer ambulance officers and paramedics felt anxious was because they infrequently attend births. “It’s because of the low number of cases that we go to,” she said. “Any jobs that are seen as low frequency we just don’t have that repetition to be able to attend a patient, reflect on what we may or may not have done with that woman’s management, and then kind of hone our skills the next time that we have a similar job.” St John WA attends about 400 obstetrics and gynaecology-related emergencies in country WA each year. Volunteer ambulance operators are not required to have a medical or paramedic degree. Instead they need to complete a training course allowing them to fill a crucial service otherwise left empty. A St John WA spokeswoman said senior emergency medical technicians undertake emergency childbirth training annually and volunteers were trained to assist in neonatal resuscitations. The training includes identifying a high-risk birth, newborn life support, and seeking backup. In a bid to improve confidence among paramedics, in May 2023 Ms Gardiner launched the State Obstetrics Referral Call — a 24-hour helpline made up of a team of midwives. Michella Hill is also developing a virtual reality training, which would allow trainees to look at the crucial steps in the first few minutes of a baby’s life.
